OSHA 300 Log Information
Private Employers
All Private Employers with more than 10 employees (full-time, parttime and temporary workers) must complete the OSHA 300 and OSHA 300A for all workplace related injuries and illnesses that meet the recordability requirement.  These files must be maintained for 5 prior years.

Sharps Injuries
Employers must record all work-related needlestick injuries and cuts from sharp objects that are contaminated with another person's blood or other potentially infectious material (as defined by 29 CFR 1910.1030). The injury must be entered on the OSHA 300 Log as an injury. To protect the employee's privacy, the employee's name must not be entered on the OSHA 300 Log
 
Public Ohio Employers
All Public Employers must complete the OSHA 300P and OSHA 300AP for all workplace related injuries and illnesses that meet the recordability requirement.  These files must be maintained for 5 prior years.  Public employers must also send a copy of the 300AP to PERRP by February 1 for previous calendar year.  The 300AP form can be mailed (download blank form below) to PERRP or submitted online.
 
 
PERRP Sharps Injury Form Needlestick Report (SH-12)
Public employers must complete and submit a PERRP Sharps Injury Form Needlestick Report (SH-12) for every needlestick or sharps injury that occurs. The SH-12 form can be mailed (download blank form below) to PERRP or submitted online

OSHA 30-hour for General Industry
March 15,22,29 & April 5  - $595
 great course for employees at all levels including supervisors new to safety or new hires responsible for general health & safety. The course covers OSHA requirements for various subjects that are most applicable to the industry; correctly locating OSHA requirements using a copy of the general industry regulations; interpreting and applying the regulation to the work environment and selecting proper PPE for different work scenarios. Students will gain knowledge through personal experience and group exercise led by instructors to identify common solutions to general workplace hazards.
